抄録: Although antihypertensive therapy is recommended for management of hypertension in pregnant
women, the criteria for initiation and maintenance of treatment have not been established. We report a case
involving a 38-year-old pregnant woman with severe hypertension and proteinuria who was referred to our
facility at 23 weeks 0 days of gestation. Ultrasonographic monitoring of fetal well-being showed that fluctuations
in the maternal blood pressure (BP) were rapidly reflected by the results of fetal umbilical artery Doppler
velocimetry, e.g., the fetal diastolic umbilical artery flow normalized when the patient’s BP decreased to 150/88
mmHg, but disappeared when the BP further decreased to 120/80 mmHg. Thus, the patient’s hydralazine dose
was adjusted according to fetal ultrasound findings. When the patient subsequently showed no response to
hydralazine administration, which was reflected by the persistent absence of the diastolic component of umbilical
artery flow in Doppler velocimetry, emergency Caesarean section was performed at 23 weeks 6 days of gestation,
and both the mother and infant showed good clinical outcomes. Our case thus highlights the value of Doppler velocimetry in monitoring patient responsiveness to antihypertensive therapy.

抄録: Objective：The purpose of this study was to develop and evaluate the Smoking Cessation Support Ability Scale to
assess nursing students’ ability of smoking cessation support.
Methods：The original Smoking Cessation Support Ability Scale was developed based on a comprehensive review of
the literature. A revised version was developed based an examination of the content validity of the original scale and
preliminary investigation. Subsequently, the revised questionnaire was distributed to 243 nursing students and
examined its reliability and validity.
Results：Exploratory factor analysis by principal factor analysis with promax rotation was conducted. The following
five factors comprised of 18 items were finally extracted: “Leadership,” “Explanation of risk of the smoking,”
“Communication,” “Planning”, and “Attitudes toward smoking.” The reliability of the scale was confirmed by a
Cronbach’s alpha internal consistency reliability coefficient of 0.77, and atest-retest reliability coefficient of 0.89. The
criterion-related validity was confirmed by Kikuchi’s Social Skill Scale-18,Self-efficacy Scale in Nursing Practice, and
the Kano Test for Social Nicotine Dependence.
Conclusion：The Smoking Cessation Support Ability Scale was confirmed its reliability and validity.

抄録: Objectives : To clarify concept of “disability” on perceptions among nurses and care workers through experiences of
clinical working fields of long-term care institutions and to assess perceptions among nurses and care workers in
care of residents with “disability”.
Design : qualitative and inductive approach by grounded theory approach
Setting : 2 long-term care institutions in A city.
Participants : 10 numbers of qualified each 5 nurses and care workers over five year careers.
Results : Perceptions among nurses and care workers to take care of residents with “disability” were classified from
differences to concordance according to the subjects. In the category of point of view in the ways of relation, nurses
and care workers expressed fluctuation and resistance in the word of residents with disabilities. They have tried to
not only take care of residents with disabilities without bias, but also treat able-bodied person as normalization.
Additionally, their underlying caring is independence, support for uncontrollable parts(functions) in ADL/IADL, and
acceptance of as a living person. Moreover, job situations for care workers were more satisfied than that of nurses.
On the other hand, it turned to be more stressful for care workers because caring for dementia is one of the most
difficult cares. However, the most stressful situations were human relations including residents’ families and doctors
rather than low wage or demanding work.
Conclusions : The findings of this study support education for new faces, and management of personnel or work and
health organizations.
